Insurance Policy
We are happy to work with you to maximize your dental benefits and will provide the necessary information for you to file for a claim for reimbursement directly from your insurance company. Your dental insurance should provide a paper form to file a claim for reimbursement, these forms are usually provided in the back of the handbook, or perhaps as an electronic PDF that you can fill in before printing. Our office requires direct payment to be made at the end of each visit, unless other arrangements have been made. Financial Policy
For your convenience, we accept payments made with cash, debit cards, Visa, MasterCard, Discover, and American Express. We also offer financing through CareCredit®.
Cancellations
Kindly provide at least 48 hours notice for any appointment that needs to be rescheduled. Appointments cancelled in less than 24 hours may be subject to a cancellation charge.
